PROVIDENCE ACADEMY 3-K AND 4-K PRE-SCHOOL TO GRADE 12 ENROLLMENT APPLICATION ACADEMIC YEAR 2026-2027

Families registering for Pre-School (3-K and 4-K) ONLY: After submitting this enrollment application, please submit the non-refundable enrollment fee of $75 per family by Monday, March 16, 2026.

Families registering for Pre-School and grades K–12, or K–12 only: After submitting this enrollment application, please submit the non-refundable enrollment fee of $150 per family by Monday, March 16, 2026.

Payment Instructions: The enrollment fee may be submitted by check mailed to the office, delivered in person, or submitted online through the Donate page of the website.

Registration Agreement

We, the parents/guardians of the student(s), acknowledge that by submitting this application, we agree to the application process set forth by Providence Academy. We understand that Providence Academy reserves the right to reject this application at any time and that submission does not guarantee enrollment. Furthermore, we affirm that all information provided about the student(s) in this application and throughout the enrollment process is accurate and complete to the best of our knowledge.
